VETERAN AND SPOUSE CARE
Veteran and spouse care is a form of home care offering non-medical services to older veterans and their spouse or veteran and their spouse who has disabilities. We partner with VCC (Veterans Care Coordination) to get the qualified veterans approved for services. The goal of veteran and spouse care is primarily personal care, emotional support and socialization and a variety of tasks including light housekeeping.
From playing games to sharing stories that stimulate the mind and keep the body active, our professional caregivers provide social interaction for lonely veterans and their spouse. The care plan will be tailored based on your loved one's unique care needs and preferences. We will match them with a friendly caregiver.
RESPITE CARE
Respite care provides temporary relief for a primary caregiver, enabling you to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member. Respite care can take place in your own home, at day-care centers, or at residential or nursing facilities that offer overnight stays.
Our trained healthcare professionals provide the family with a short term break from ongoing caregiving responsibilities.
PRIVATE DUTY NURSING
Private duty nursing (PDN) is hourly, skilled nursing care provided in a patient's home. Private duty nursing provides more individual and continuous skilled care than can be provided in a skilled nurse visit.
Our private duty nurses take the time to make sure our clients’ safety and medical needs are met. The goal of our private duty nurses is to help individuals stay within their homes and live happy and healthy lives among relatives and friends.
COMMUNITY GUIDE SERVICES
Community guide services include direct assistance to promote individuals' self-determination through brokering specific community resources that lead to connection to and independent participation in integrated, independent housing or community activities so as to avoid isolation.
Our team members provide resources to individuals based on their wants, interests and abilities. The community guide services is to support long-term independence and allow for a better quality of life for our clients.
IN-HOME SUPPORT SERVICES
Looking for more than just the personal care for you or your loved ones? In-home support allows individuals to receive all the ADL and IADL services, plus additional services like skill building.
In-home support services is designed for individuals of all ages with intellectual disabilities living in the their own home or with their families. It covers supervision; Training; Assistance with personal care and Community integration.
